Growing families deserve a healthy start. 


Smart Start Partnership for Children is proud to partner with the Henderson County Department of Public Health to offer Family Connects

Family Connects is a nurse home visiting program for families with newborns in Henderson and Polk counties. Families will receive a home visit from a highly trained, registered nurse who is knowledgeable about the needs of babies and their caregivers. Our nurses will provide health screenings and guidance on baby care to help caregivers feel confident in their abilities. The nurse will also connect families to resources that anyone in the household may need, facilitating referrals to community services that will help the household thrive.

 postpartum health check · breast or bottle feeding help · postpartum depression screening · understanding childcare options
understanding new family dynamics · back-to-work support · help with bathing, diapering, and swaddling · safe sleep information
management of infant crying · financial resources · playgroups and parent support groups · early literacy information
family planning advice · scheduling appointments · healthy home connections 


 Family Connects is evidence-based. Research shows that every family needs support after the birth of a child. Smart Start works in partnership with Family Connects International to support our families as they grow by offering nurse home visits and connection to community resources.
