 Smart Start Partnership for Children seeks a Family Connects Nurse Home Visitor.

A Family Connects home visiting nurse provides virtual or in-home visits to families with newborns in the Henderson and Polk Counties. The goal is to support and assess family needs and facilitate referrals and connections to community agencies to support families. The home visiting nurse will work in a team environment of clinical and nonclinical staff to schedule, visit, and support families in the community using the evidence-based Family Connects home visiting protocol.

Bilingual (English/Spanish) Data and Program Support Specialist 

 Smart Start Partnership for Children seeks a Bilingual Family Connects Data and Program Support Specialist. This position works with a team of nurses to coordinate Family Connects visits, schedule and coordinate appointments, and communicate with families. This position also oversees data collection, reporting and analysis. A successful candidate will have excellent communication skills in English and Spanish, strong attention to detail and be passionate about meeting the needs of families with newborns.
